She must have felt like she wasnt getting any attention for a few moments.

They've told her she can get lots and lots of money if she does 'risqué' things. She's admitted in interviews that it's all about the money. Shocking the audience is what gets views/attention, and in today's world, views = $$$… literally.
She's not doing anything else much different than the rest of her peer group is doing these days, getting tattoos all over, taking ridiculous pictures with ridiculous faces and poses, dressing for shock value, etc. The only difference is that she's famous, so her displaying this behavior will in turn spread it.
The only thing that will make what she's doing "worth it" is if she's able to hold onto that money decades down the road. I'd hate to be Miley Cyrus and have nothing to show for it at age 40.
Of course if she starts doing porno, then it's doubtful that she'll even make it to 40, and it's definite that she'll be in the poorhouse if she does.
